Risikon palvelumalli sopii Pirkanmaalle
1. Paras-hankkeen tavoitteeksi asetettiin 2005 kuntapalveluiden ja kuntajaotuksen muutoksien sekä kuntarajat ylittävän yhteistyön tarkastelu.
2. Uudistusten tavoitteena on kehittää palveluiden tuotantotapoja, uudistaa rahoitusjärjestelmiä, tarkistaa valtion ja kuntien välistä tehtävien jakoa sekä parantaa tuottavuutta ja hillitä menojen kasvua.
3. Paras-hanke ei ole kuitenkaan saavuttanut sille annettuja tavoitteita eli laadukkaiden palveluiden varmistamista asukkaille.
Uudistuksen puitelain mukaan kunnassa tai yhteistoiminta-alueella, joka huolehtii perusterveydenhuollosta tai siihen kiinteästi liittyvistä tehtävistä, on oltava vähintään noin 20 000 asukasta.
Paras-hankkeen seurauksena Pirkanmaalla on tämän vuoden alusta 24 kuntaa ja kymmenen yhteistoiminta-aluetta. Nämä alueet ovat yhden tai useamman kunnan muodostamia perusterveydenhuollon palveluita tuottavia kuntayhtymiä tai isäntäkuntamalliin perustuvia kuntien yhteenliittymiä.
Pirkanmaan sairaanhoitopiirin (PSHP) tehtävänä puolestaan on tuottaa terveydenhuollon palveluja sekä luoda edellytyksiä tätä tukevalle tieteelliselle tutkimukselle ja koulutukselle. Kuntien kanssa tehdyt palvelusopimukset määrittelevät tuotettavien palveluiden määrän ja valikoiman. Lisäksi yliopistollinen sairaala tuottaa palveluja erityisvastuualueensa sairaanhoitopiireille.
Valtakunnallisesti Paras-hanke ei ole saavuttanut sille annettuja tavoitteita eli laadukkaiden palveluiden varmistamista asukkaille. Sen sijaan se on luonut uusia hallintorakenteita, joiden myötä päätöksenteko on luisunut joissakin tapauksissa kauemmaksi peruskunnista.
Osittain tästä syystä peruspalveluministeri Paula Risikko (kok) esitti elokuun lopussa sosiaali- ja terveyspalvelujen järjestämiseen uutta mallia, joka mullistaisi melkoisesti nykyistä käytäntöä.
Risikon lähtökohtana on, että nykyiset sosiaali- ja terveydenhuollon rakenteet eivät riitä vastaamaan tulevaisuuden haasteisiin. Tarvitaan nykyistä selkeämpiä ja vahvempia rakenteita, koska esimerkiksi nykyinen perus- ja erikoissairaanhoidon rajanveto aiheuttaa sekä toiminnallisia että taloudellisia ongelmia ja sitä voidaan pitää myös keinotekoisena.
Ministeri Risikko lakkauttaisi nykyiset sairaanhoitopiirit ja rakentaisi 40-60 kokonaan uutta sosiaali- ja terveysaluetta. Ne olisivat riittävän vahvoja sekä toiminnallisesti että taloudellisesti. Kukin alue järjestäisi sosiaali- ja terveydenhuollon peruspalvelut ja valtaosan erityispalveluistakin. Nykyiset aluesairaalat kuuluisivat sosiaali- ja terveydenhuoltoalueisiin ja tuottaisivat peruserikoissairaanhoidon palveluita omalle alueelleen.
Maa jakautuisi edelleen viiteen yliopistosairaalajohtoiseen erityisvastuualueeseen, jotka olisivat kuntayhtymiä. Nämä erva-alueet vastaisivat, kuten tähänkin asti, vaativasta erikoissairaanhoidosta, kuten elinsiirroista, keskoshoidosta, vaativasta syövän hoidosta ja palovammojen hoidosta, monivammahoidosta sekä uudessa mallissa joistakin sosiaalihuollon erityispalveluista, kuten vaativasta päihdehuollosta. Nykyiset keskussairaalat tuottaisivat alueelleen erikoissairaanhoidon palveluita sopimuksen mukaan.
Risikon esittämä sosiaali- ja terveydenhuollon järjestämismalli soveltuu Pirkanmaalle. Pirkanmaalla on tällä hetkellä kymmenen terveydenhuollon yhteistoiminta-aluetta. Meillä on yliopistollinen sairaala sekä kaksi aluesairaalaa (Vammala ja Valkeakoski) sekä Mäntän terveydenhuoltoalue.
Pirkanmaalla Vammalan ja Valkeakosken aluesairaalat tuottaisivat kukin omalle sosiaali- ja terveydenhuoltoalueelleen peruserikoissairaanhoidon palvelut. Mänttä-Vilppulassa toimii jo Risikon esityksen mukainen terveydenhuoltoalue, jossa perusterveydenhuolto ja peruserikoissairaanhoito toimivat samassa terveydenhuoltoalueessa. Siihen kuuluu myös Ruovesi.
Tampere on yksin riittävän suuri muodostamaan oman alueensa Hatanpää keskuksenaan. Yliopistosairaala Tays voisi nykyistä kiinteämmässä yhteistyössä Hatanpään sairaalan kanssa vastata Tampereen erikoissairaanhoidosta sekä erityisvastuualueen vaativasta erikoissairaanhoidosta ns. miljoonapiirin alueella.
Suurimman haasteen tuo sosiaalipalveluiden liittäminen osaksi uusia alueita. Joskin Pirkanmaan kunnista moni on jo yhdistänyt sosiaali- ja terveydenhuoltonsa.
Ministeri Risikon esittämä malli toki tarvitsee vielä säätämistä ja sen soveltaminen koko maahan ei onnistune yhtä helposti kuin Pirkanmaalla. Se sisältää kuitenkin hyvän ratkaisuyrityksen terveydenhuoltomme keskeisimpiin haasteisiin erikoistason hoidon paremmasta järjestämisestä sekä perusterveydenhuollon järjestelmän vahvistamisesta alueelliselta pohjalta.
Tällöin kroonisten kansansairauksiemme hoitoon pystytään rakentamaan saumattomat hoitoketjut ehkäisevästä työstä aina erityistason erikoissairaanhoitoon ja sen nykyistä vahvempaan keskittämiseen asti.
Tulevaisuuden haasteet vaativat uutta näkemystä ja rohkeutta tarttua toimeen.
